STEP 1: FIND OUT IF YOU MAY BE ELIGIBLE
Take this short test to see if you may be eligible for a Medical Registry ID Card under Florida regulations. We’ll email you the results!
STEP 2: INITIAL APPOINTMENT
Schedule an in-person assessment at a location near you by calling 813-669-4045.
Your provider will work with you to identify treatment options to suit your individual needs. If you do not qualify there is no charge for your appointment.
During your appointment, your provider can help you to apply for your Registry ID Card with your state office. We will provide you with a copy of the application and any personalized assistance you need.
STEP 3: PICK UP YOUR RECOMMENDATION
When the state has processed and approved your application, you can visit a licensed facility to pick up your recommendation or schedule it to be discreetly and securely delivered.

STEP 4: FOLLOW-UP APPOINTMENT
Florida law requires that a patient meet with their provider two times per year. You will meet with your provider in person for a follow-up assessment. Your medical team can be reached at any time to answer questions and assist you with your treatment. Contact your patient care coordinator or medical professional at 813-669-4045.
